Referral Process
When making a referral, we would greatly appreciate it if you could provide Berks Counseling Center (BCC) with the following information:
Please call our Admissions Clinical Care Coordinator at 610-373-4281, Ext. 2726, or email us at intake@berkscc.org.
Please include the following details:
- Patient's level of care (e.g., Mental Health, Drug & Alcohol, Dual Diagnosis, Intensive Outpatient Program, or Intensive Outpatient Program Dual Diagnosis)
- Demographic information
- Insurance information (please note that if the patient has private insurance, it may take up to two days to verify)
If the patient is currently receiving inpatient services, please also provide the discharge paperwork or the most recent psychological evaluation by faxing it to our confidential number at 610-373-3779.
